RELEASE AND WAIVER OF LIABILITY AGREEMENT. I acknowledge that I have voluntarily applied to participate in activities with the10futbol for tryouts, games, practices, travel (domestic and international), and competitive tournaments. I am aware these activities are hazardous and could result in serious injury or death. I voluntarily participate with full knowledge of the risks and agree to assume all risks of bodily injury, death, or property damage, whether known or unknown.

As consideration for being permitted by the10futbol, the State of California, the County of Sacramento, and any lessor of the premises, I forever release the10futbol and all related parties (“Releasees”) from any and all actions, claims, or demands…

Photography Release Form

I hereby authorize The10futbol to edit, change, copy, and use photographs/videos of me/the participant for promotional purposes. I understand I am not entitled to payment or compensation and that such materials may be used lawfully on the internet, magazines, books, etc.
